Troubling student behavior or speech

Are you concerned about a student's physical or mental health? Has a student's behavior caused you to worry about his or her potential actions? Become aware of the warning signs and if needed, complete the Assistance & Referral form to inform the appropriate university personnel.
 

Troubling student academic work

Have you identified factors that are interfering with a student's ability to be academically successful? Could early intervention with academic resources make a difference? Complete the Academic Alert form to notify the assistance team.
